Complete your telehealth evaluation
A Georgia-licensed provider will review your medical history via a short video call. If approved (99% are), your certification is submitted directly to the Georgia Department of Public Health.
Pay the state fee & receive your card
The Georgia DPH will email you a payment link for the $33.75 state fee. Your physical card is mailed to your ID address — usually within about a week. No mailing address? It will be held at your local health department for pickup.
